 f &amp\; Hard of Hearing Service Center\, Inc. (DHHSC) at 339 Pajaro Stree
 t\, Suite B\, Salinas\, CA 93901 and pick up a form.  Take the form to on
 e of the certifying agents listed on the form and have them complete the c
 ertification.  Bring that certified form to DHHSC on the second Friday of
  the month (except holidays) to receive their FREE phone and/or telecommun
 ications accessories.\nIf you currently have a CTAP phone and have questio
 ns about it or are having a technical difficulty with it\, bring the compl
 ete phone and/or accessories to the Service Center for instruction\, troub
 leshooting and if needed replacement under CTAP guidelines.  If you have
