For new, first time, clients to CTAP, they will need to bring a completed and certified application form with them to receive a phone on the day of the service center. Here is the web site where they can print out an application form www.californiaphones.org or they can come by Deaf & Hard of Hearing Service Center, Inc. (DHHSC) at 339 Pajaro Street, Suite B, Salinas, CA 93901 and pick up a form. Take the form to one of the certifying agents listed on the form and have them complete the certification. Bring that certified form to DHHSC on the second Friday of the month (except holidays) to receive their FREE phone and/or telecommunications accessories.
If you currently have a California Telephone Access Program (CTAP) phone and have questions about it or are having a technical difficulty with it, bring the complete phone and/or accessories to the Service Center for instruction, troubleshooting and if needed replacement under CTAP guidelines.
